Description: Heart failure (HF) is frequently associated with other chronic conditions (such as ischemic heart disease, diabetes, chronic renal disease, atrial fibrillation, obesity, and hypertension), leading to a complicated web of health concerns. Interconnectivity expresses itself in a variety of ways, influencing the diagnosis, detection, and treatment of these illnesses. Community pharmacists and their teams are uniquely positioned to improve HF awareness, detect those at risk, and drive early identification and referral to primary care providers for assessment, diagnosis, and intervention. At this event, the "Heart Failure Pharmacy Toolkit" was presented to assist pharmacists offer a range of HF pharmacy treatments.
